Personal Support Services

These are habilitative services that assist people who live in their own or family homes with acquiring and building the skills necessary to maximize their personal independence.

These services include:

In home skills development
Community integration and engagement skills development
Personal care assistance services

Enhanced Personal Supports may be provided, based upon a person’s needs when:

− The person has an approved Behavioral Plan; and/or

− The participant has a Health Risk Screening Score of 4 or higher.
