Most frequent questions and answers

We provide Targeted Case Management to individuals with developmental disabilities. This is also referred to as Resource Coordination or Coordination of Community Services*.  If you have a developmental disability, we will  work with you to develop a plan that suits your needs. Once  your eligibility is determined and you have a plan, we will help you navigate the system to access Home and Community-based Services such as:

  1.  Residential / Personal Support Services
  2.  Day /Employment Services
  3.  Support Services

Our services include:

  1. Eligibility assessment for DDA services
  2. Development of an Individualized Person Centered Plan
  3. Referral to various services and resources in the community
  4. Transitioning Youth Services
  5. Transition Coordination Services (from institution to community)
  6. Obtaining funding for DDA services
  7. Monitoring and follow-up

 

We recognize that there is a need for specialized care for the people we serve and it is our priority to ensure that there is continuity in services from school through adulthood..

If someone you care about has been diagnosed with a developmental disability, you may be concerned about how to plan for the future and who will guide you through the process. The Maryland Developmental Disabilities Administration funds a system of services to provide people with developmental disabilities and their families guidance and support. Please apply for services as soon as you are aware of a significant disability.  Eligibility can be delayed because it is difficult to predict how a child will continue to develop, and what milestones children might reach as they grow. However, applying as soon as possible, allows the DDA to be aware of your child’s needs, and if approved, you will have a case manager (CCS) for a partner as your child develops.  The application is on the website for Maryland DDA.  When you send in the application, please include copies of any assessments and IEPs from their school. If you need help with the application, please contact our Information and Resources team.

 If you would like to work with us, please indicate this on the Choice Form. You can also call the DDA office to make your choice.

Your Coordinator of Community Services or CCS will help you locate the services you qualify for, and assist you in the application for those agencies. Services being provided are determined by eligibility and need. Some of the services provided by DDA funding are as follows:

Coordinators of Community Services (CCS) assist eligible individuals and their families in  developing plans and finding the services to fulfill them.   Your CCS is a partner, a navigator you can rely on  to listen carefully to your concerns and dreams, be responsive to your requests and find the answers you need. With the assistance of your CCS, you will be able to identify your goals, create a plan that works for you, and access services or activities that make sense for you and the lifestyle you choose.

We partner with DDA to better serve you and your loved ones.

One of the most important times in a young person’s life is the transition from school to adult services. The types of services offered are very different, and the eligibility and timeline for adult services are structured differently than school. 

Your CCS will assist you and the IEP team in planning and applying for the services needed after graduation. This process starts early in high school, and requires planning and follow up from the student and their family. In order to be eligible for TY services in Maryland, the following criteria must be met:

  • Maryland resident
  • 21 years old 
  • DD eligible (DDA determines this)
  • Eligible for Medical Assistance
  • Your CCS will assist you in completing a Waiver Application by March of the final school year, and working with your student’s adult provider to submit the documents needed to DDA
  • Once the plans are approved, you will receive a letter from DDA with a start date for services.

As your student is getting ready to graduate from school – 

AGE 19      

  • APPLY FOR DORS 
  • ATTEND A GTYI FAIR
  • VISIT AT LEAST  3 DDA SERVICE PROVIDERS

AGE 20      

    • By December : SELECT  Day  Service Provider & COMPLETE the Application
    • By March 31st  : CCS completes WAIVER APPLICATION & Matrix Request 
  • By April – May  : DAY PROVIDER submits SERVICE FUNDING PLAN to DDA

By June : DDA LETTER to you with  SERVICE START DATE

There are many details in determining the right services for you and your family. Your Coordinator of Community Services (CCS) is your partner, advocate and guide while you get through this process. The CCS will continue to meet with you at regular intervals, provide the information you need, and be your “go to” for information, ideas and support.

Additional tips:

  • Help your child to develop independent vocational skills at home by assigning and teaching chores that must be completed on schedule with your guidance 
  • Attend IEP meetings and keep the documents for use with applications
  • PLAN for the future by asking the CCS about a special needs trust, ABLE account, surrogate decision making or other specialized planning. Remember that your student’s eligibility will partially be determined by their income/assets.

People with disabilities can choose who they live with, how they spend their day, participate in the community in ways they choose, and spend time with people important to them. Self-direction gives waiver participants and families greater control over the services they receive, how they receive them, and who provides them. At the same time, people who self-direct must be willing to take on the responsibility of managing their services.

  • You will be in control of your budget for the purchase of services and supports 
  • You select and arrange for the services and supports in your individual plan
  • You are accountable for using your budget to more effectively meet your needs 
  • You can hire, train, and fire employees

Your CCS will assist you in choosing your services and also connect you to a Support Broker and a Fiscal Management Service to support you.

Self-Directed Support Services also include:

  •  Fiscal Management Services
  •  Support Broker Services (optional service)

Transition Coordination Services are provided for people actively transitioning from an institution to the community, including people residing in a State Residential Center (SRC) that do not attend an off-campus day program, people in nursing facilities, and people in the Secure Evaluation and Therapeutic Treatment (SETT) Programs.

 

“Forensic” means related to, or associated with, legal issues. Forensic services provide assessment and treatment of people with a I/DD and a history of criminal offending or those who are at risk of offending. Forensic services aim to support the person with living a successful life in their community and reduce the risk for relapse of offenders with I/DD and other complex co-occurring needs.

Court orders must be followed in order to facilitate their transition from a facility to community-based programming.

 

The CCS is –

  • Responsible for comprehensive assessments
  • Monitoring and follow‐up activities
  • Referral and related services
  • Development and periodic review of

Person-Centered Plans for individuals who

have forensic involvement.

Funding designed to improve an individual or family’s quality of life, increase or maintain independence, and participate in their communities. The LISS program uses an automated system called the Random Selection Process to select individuals who may be eligible for funding, granting up to $2000 for services and items to address their needs. The DDA distributes LISS funding twice during the fiscal year with applications accepted during Round 1 and Round 2.

 

  • It is flexible to meet the needs of children as they grow and adults as they mature across their lifespan
  • Provides up to $2000 to assist eligible children and adults with developmental disabilities with purchasing eligible services and/or items to address their needs.
  • Enhances or improves the quality of life, for eligible children and adults with developmental disabilities as well as promoting personal independence and community integration.​
